The Health Improvement Partnership -Cuyahoga took another step closer to the completion of the Health Improvement Plan for Cuyahoga County at the Strategic Issues Conference on March 21st.

Over 45 planning partners gathered to utilize a health equity framework to select strategic issues and a set of key priorities.

The conversation was led by a team from CommonHealth ACTION, a national non-profit public health organization that works to build the capacity of communities and organizations to create equal opportunities for all people to achieve optimal health.

Highlights from the 2012-2013 Community Health Status Assessment Report were shared. The full report,along with an executive and high level summary are available here.

Two overarching strategic issues were established and influenced the selection of key priorities. The HIP-C went through a group process to reach agreement on the top key health priorities facing Cuyahoga County from a lengthy list that had come out of earlier HIP-C planning efforts. These four priorities will be targeted for action over the next two years.

Improve chronic disease management through the engagement of various sectors

Improve coordination between  Clinical Care and Public Health to improve population health

Eliminating Racism as a Social Determinant of Health

Increase access and opportunity for improved nutrition and physical activity

Next Steps include:

HIP-Cuyahoga will be hosting up to 6 community conversations to gather input on priority areas for action buy Cuyahoga County residents, and to involve community members in the HIP-Cuyahoga process.

A planning event will take place in July  to discuss what we want to achieve by addressing the four key priorities, and how we want to achieve it.

Revision of the Forces of Change Assessment

A key component in determining our eventual action steps, the Forces of Change assessment was created during the July Planning Committee session.  Based on community improvement plan best practices, several discussion groups reviewed various themes, opportunities, threats and challenges facing our regional public health system and the community at large.

Task at Hand

Your help is requested in examining the list created by the Planning Committee and identifying the top ten forces impacting our region.
